Registering

Please enquire at reception where our staff will assist you in making the necessary appointments and completing the appropriate documentation.

A new patient health check is required for all new patients registering with the practice. Please bring a fresh urine specimen with you for this appointment. Specimen containers are obtained from reception when applying to register.

Please state all illness and medical problems on the registration form, as failure to do so may result in rejection of your application. Do also ensure you attend your new patient health check as failure to do so may also result in rejection of your application.

Once accepted onto our register, you have the right to express a preference regarding which Clinician you can see. However, this may result in waiting longer for an appointment.

Every patient who is on our register will be required to attend an annual health check. If a patient aged between 17 and 75 has not consulted a clinical member of staff at our practice within three years of registration, an appointment will be arranged for a general health check. Examination and enquiries will be made in all circumstances. If a patient has reached the age of 75 years and has not been seen by any clinical member of staff at our practice within 12 months of registration, an appointment will be arranged for a general health check. Examination and enquiries will be made in all circumstances. If it is deemed necessary, the patient can be visited at their home for this consultation.
